A Comprehensive Medication Review is a review of a patient’s entire medication regimen for the purpose of identifying and eliminating medication-related problems. Patients should have follow-up reviews every 3 months or when major changes are made to their medication regimen
Uses prescription medications, non-prescription medications, herbals, or other dietary supplements
Has several health conditions
- Has questions or problems with their medications
Is taking medications that require close monitoring
Has been hospitalized or experienced transitions in care (i.e. Hospital–>Home)
Obtains their medications from more than one pharmacy.
